Patients at risk of blood clots may be prescribed blood-thinning medication to prevent clots from forming. SOIM’s Coumadin Clinic helps monitor those taking anticoagulation medication such as Coumadin. Our staff works closely with patients and their physicians to maximize the benefits associated with anticoagulation medication while reducing adverse risks.

Located on the first floor of the Rogue Valley Physicians medical building, and across the street from Rogue Valley Medical Center.

DIRECTIONS

Take I-5 to the South Medford exit #27.
Turn east onto E. Barnett Road. Travel approximately one
mile to Murphy Road. Turn left (north) onto Murphy Road.
Turn right onto Doctors Park Drive. RVP is the first building
on the right side of the street. Ample parking is available.
The Coumadin Clinic is located on the first floor.
